Use these forms from the Florida Department of Highway Safety and Motor Vehicles to register your vehicle, vessel and mobile home liens.

ELT DMV ID Number
FLHSMV Form 82150 - Electronic Lien & Title DMV ID Number
The DMV form 82150 must be completed by the lienholder after registering for Florida Electronic Lien and Title service. It allows the lienholder (car dealer, bank, credit union, finance company) to obtain what's known as an ELT DMV ID Number. Once the 82150 form is submitted you can expect to be able to manage your Florida electronic liens within a few days (often sooner). Easy instructions are included in the file.

TITLE APPLICATION
FLHSMV Form 82040 - Title Application
The DMV form 82040 is commonly used by dealers when selling a vehicle, vessel or mobile home and placing a lien in their business name or another finance company's name. We encourage you to read the form completely for instructions, but generally the dealer registers the title in the name of the customer and uses section 4 to add the lienholder's information, including the required DMV ID Number (if you're a lienholder and don't have an ELT DMV ID Number, register for Florida ELT service and then complete DMV form 82150).

NOTICE OF LIEN
FLHSMV Form 82139 - Notice of Lien
The DMV form 82139 is used by lienholders to place a lien on a vehicle, vessel or mobile home. This form is most commonly used by lienholders that are NOT the dealership acting as the seller. We encourage you to read the form completely for instructions, but generally the lienholder uses section 2 to add the lienholder's information, including the required DMV ID Number (if you're a lienholder and don't have an ELT DMV ID Number, register for Florida ELT service and then complete DMV form 82150).
